How to get your crash report in Illinois

Not in Illinois? Pick your state

The report names everyone involved and records what happened. Insurers rely on it, and any claim you make is built on it.

  1. 1

    Know what it's called

SR 1050, the Illinois Traffic Crash Report

Every police agency in the state must use it. Drivers no longer fill in their own Motorist Report, but if no officer came to a crash with an injury, a death or over $1,500 of damage, the driver must still tell the police right away.

  2. 2

    Find where to request it

State Police crashes, mostly interstates, tollways and state roads: the Illinois State Police

Order it online without an account, or by mail to the Patrol Records Unit in Springfield. Have the crash report number, a driver's name, the date, the location and the county ready. A crash reconstruction report is by mail only.

Anywhere else, including Chicago city streets: the police department or sheriff's office that responded

Ask whoever wrote it. The State Police rarely handle crashes on city streets, but they do work Chicago-area expressways and tollways.
