How to get your crash report in Iowa

Not in Iowa? Pick your state

The report names everyone involved and records what happened. Insurers rely on it, and any claim you make is built on it.

  1. 1

    Know what it's called

Investigating Officer's Report of Motor Vehicle Accident

It has no form number, and almost every one is filed electronically. Drivers file a different report of their own, Form 433002, within 72 hours when someone is hurt or killed or damage reaches $1,500, unless police investigated.

  2. 2

    Find where to request it

    Most crashes: Iowa DOT

Fill in the DOT's online request form, then print and sign it. Mail it with the fee to the Bureau of Enforcement, Audits, & Records, Iowa Department of Transportation, P.O. Box 9204, Des Moines, IA 50306-9204.

Or: the police department or sheriff's office that responded

It may keep its own copy. Ask it directly.

If a State Patrol trooper came to your crash, a short version of the report is free to view online for 15 days. It is not the full report.
