How to get your crash report in New York

Not in New York? Pick your state

The report names everyone involved and records what happened. Insurers rely on it, and any claim you make is built on it.

  1. 1

    Know what it's called

    MV-104P, the Police Crash Report

Every police agency in the state uses it. Some reports, including some from 2026, still say MV-104A, or MV-104AN in New York City.
