How to get your crash report in Oregon

Not in Oregon? Pick your state

The report names everyone involved and records what happened. Insurers rely on it, and any claim you make is built on it.

  1. 1

    Know what it's called

735-46, the Oregon Police Traffic Crash Report

Police use it statewide. It is a different report from the one drivers file themselves.

  2. 2

    Find where to request it

    Most crashes: the Oregon DMV

Mail form 735-7266 with a check or money order, ticking the police traffic crash report box. Give the crash date and county, and the address to mail it to.

In Portland: the Police Records Division

Request it through the city's online records request center, choosing the Police Bureau, or mail a request with a check or money order.

Drivers must also file their own report with the DMV within 72 hours in some crashes, such as when someone is hurt or your car has over $2,500 of damage. The DMV does not give out copies of that report.

Neither fee is refunded if no report is found. Portland also charges for staff time past the first 30 minutes.
